By participating in the Home Makeover Sweepstakes, I agree to be subject to these Terms and Conditions.  Submission of this entry form serves as permission for Universal Windows Direct, LLC and its affiliates (collectively, “Universal Windows Direct” or “UWD”) to contact me via a phone or email listed above regarding a no-obligation in-home estimate on home improvement products. I am also interested in hearing about future promotions, goods, and services offered by Universal Windows Direct. I understand that my contact information will NOT be distributed to any third parties except as set for herein (including the attached Privacy Policy) or otherwise authorized by me.

TERMS AND CONDITIONS

1. This is an annually reoccurring sweepstakes promotion, with four (4) prizes being awarded for each sweepstakes. A sweepstakes begins on January 1, 2013 and ends on December 31, 2013 of each year. Universal Windows Direct, Inc. ("UWD") or its designee will award one (1) prize at the end of each prize period. The prize is a selection of up to $5,000 in approximate retail value of installed UWD exterior home remodeling products. The prize periods expire on April 1, July 1, October 1 and December 31 of each year. Void where prohibited by law, rule or regulation. Each prize winner will be determined from a random drawing of all entries received during a sweepstakes, excluding prior winners. Drawings will be held within five business days of the end of each prize period. The odds of winning any prize are determined by the number of entries received prior to the time of the prize drawing. UWD reserves the right at its sole discretion to suspend, modify or terminate this promotion without notice at any time. All prizes will be awarded.
2. No purchase is necessary to participate in this promotion, and you do not have to submit to a sales presentation. Void where prohibited by law, rule or regulation. You may enter this promotion by completing the entry form online or by mailing it back to UWD at 7813 1st Place, Oakwood, OH 44146, attn: Chris Jarvis. Alternatively, you may print your name and your complete address on a 3"x5" card, write the words "Universal Windows Direct Exterior Home Makeover Sweepstakes" at the top, and mail the card to UWD at their address. Mailed entries must be postmarked no later than 72 hours prior to the expiration of a prize period to be eligible for that prize drawing. No more than one entry per household is allowed. UWD is not responsible for any illegible, misplaced, damaged or late entries. The address listed on each entry form must be the address where the prize is to be installed if such entry is selected as a winner.
3. Sweepstakes entrants must (i) be legal residents of the Indianapolis Metropolitan Area and surrounding communities; (ii) be eighteen (18) years of age or older, (iii) own their own home, which home has a structure that is in suitable condition (as determined by UWD or its designees) for installation of the prize (including compliance with local building codes and regulations), be a single family home, and (iv) reside in a geographic area in which UWD, its affiliates, subsidiaries, co-branded partners, consultants, contractors, advertising, public relations, promotion, fulfillment and marketing agencies, their immediate families (and those living in their same households) are not eligible to participate in the promotion.
4. Each prize winner will receive the winner’s choice of up to Five Thousand Dollars ($5,000) approximate retail value of UWD products, professionally installed by UWD or its designees. Any costs in excess of approximate retail value of the prizes shall be the sole and exclusive responsibility of the winner. Prizes are not transferable by the winner and must be installed at the address listed on the winning entry form. Installation must be provided by UWD personnel or UWD's designee. Prize subject to availability and may be subject to black-out dates. Installation subject to the terms and conditions of UWD's standard installation guidelines. All expenses not specifically provided for are the sole responsibility of the winners.
5. Winner's home must be in suitable condition for products installation (e.g., no water, mold, structural damage, etc.). Any waterproofing, remediation or other repairs necessary for proper installation (as deemed by UWD or its designees in their sole discretion) must be performed at owner's expense prior to installation of the products. Installation labor does not include painting, staining, floor coverings, lighting or HVAC systems. No transfers, substitution in prizes or cash will be allowed except by UWD, who may substitute a prize of equal or greater value in its sole or absolute discretion. Prizes awarded have no cash value. Upon installation of prize, UWD will be deemed to have awarded the prize to the winner with the winner assuming full ownership and responsibility for the prize. Winner must arrange for prize installation no later than one month following the award of the prize and installation must occur and be completed within six months of confirmed notification.
6. The winner must comply with all instructions in the prize notification correspondence, which may include the completion of an affidavit of eligibility and liability/publicity release, as well as have met all of the eligibility requirements set forth within these Rules; otherwise prize will be forfeited and an alternate winner will be selected. Acceptance or use of a prize constitutes permission for UWD and its affiliates to use recipient's full name and likeness for advertising and promotional purposes without compensation, unless prohibited by law. By accepting this prize, the winner agrees that UWD and its affiliates may use the winner's full name, take photographs and video of the winner and his/her family, before and after photographs of the home, and photographs of the remodeling project in progress. Acceptance of the prize constitutes agreement by the winner that UWD may also interview the winner. The winner understands that UWD intends to use their full name, the photographs, video and interviews for the purpose of internal and external promotional materials, marketing and advertising initiatives. Such photographs, videos, and interviews will not be sold to other parties. Promotional materials bearing these videos, photographs or interviews may be posted on UWD or other websites and may be used in any advertising medium, including the Internet, radio and television. UWD reserves the right to use any video, photographer interview for a time period beginning when acceptance of the prize occurs and acceptance of the prize provides UWD with unlimited use of these items.
7. Liability for any applicable taxes, including federal, state, and local taxes and/or other fees that may apply to prizes are the sole responsibility of the winner. Any required tax reporting forms will be furnished to the Internal Revenue Service. All expenses not specifically provided for by UWD in these Rules are the sole responsibility of the winner.
8. Entries that are lost, late, illegible or incomplete for any reason will not be eligible. UWD reserves the right to modify or terminate this Sweepstakes at any time or disqualify any entry in good faith. In the event of a dispute concerning the identity of the person submitting an entry, the entry will be deemed to be submitted by the person in whose name the winning home is titled.
9. By participating in the promotion, participants agree to release, discharge and hold harmless UWD, its affiliates, co-branded partners, subsidiaries, retailers, agencies and their respective officers, directors, employees and agents, from any damages which may arise out of participation in the promotion in any way or out of acceptance, use, misuse or possession of a prize. Acceptance of a prize may require entering a standard contract or purchase order for the goods and services to be provided, as required by state law. This promotion is conducted in English (which will be given its everyday ordinary meaning).
